General Information:

Midazolam is a type of benzodiazepines and it belongs to the category of hypnotics and sedatives drugs. It is an ultra short benzodiazepine having the benefit of rapid onset of action, recovery, and non painful induction and lack of venous irritation.

Midazolam  (marketed in many countries under brand names Dormicum, Hypnovel, and Versed) is a short-acting drug in the benzodiazepine class that is used for treatment of acute seizures and for inducing sedation and amnesia before medical procedures.

INDICATION:


    Preoperative sedation/anxiolysis, induction of general anesthesia.

    Sedative hypnotic for gastrointestinal endoscopy, bronchoscope, dentistry and surgery done under regional anesthesia.

    In ICU for artificial ventilation which requires long term sedation.

    Patients with acute narrow angle glaucoma.

    Intrathecal or epidural administration.


DORMICUM SIDE EFFECTS:

Side effects of Dormicum are:

   *Decreased tidal volume,

   *Respiratory rate decrease

   *Apnea

   * Variations in blood pressure and pulse rate.

 In paediatrics, on Intra Venous (IV) administration desideration

   * Hypertension.

   *Paradoxical reactions.

   *Kernicterus.

   *Hiccoughs.

   *Seizure like activity and nystagmus are observed.

In adults, on IM administration

   * Headache.

   *Hiccoughs.

   *Nausea.

   *Vomiting.

   *Coughing.

   *Over sedation.

   *Headache.

   *Drowsiness local effects are observed.

 

DOSAGES:

Standard Intra Muscular (IM) dose is 0.07 to 0.08 mg/kg, i.e. 3.6 mg of midazolam.

The standard Intra Venous (IV) dose is 0.01 to 0.07 mg/kg i.e. 2.5mg of midazolam.

For induction of general anesthesia, the recommended incremental dose is 0.1 to 0.5 mg/kg following an initial dose of 0.08 mg/kg.

PRECAUTIONS:

Less frequent or lower doses should be instituted in higher risk adult and pediatric surgical patients, elderly, debilitated patients, patients with COPD, patients with chronic renal failure and CHF, patients with uncompensated acute illness, such as severe fluid or electrolyte disturbances. Use in Pregnancy or during lactation is only recommended if the potential benefits far outweigh the potential risk. Titrate slowly in adult or pediatric patients when indicated for sedation/ anxiolysis/amnesia. After a long-term administration, do not discontinue abruptly.

WARNING:

Midazolam must never be used without individualization of dosage particularly when used with other medications capable of producing central nervous system depression. Prior to the intravenous administration of midazolam in any dose, the immediate availability of oxygen, resuscitative drugs, age- and size-appropriate equipment for bag/valve/mask ventilation and intubation, and skilled personnel for the maintenance of a patent airway and support of ventilation should be ensured. Patients should be continuously monitored with some means of detection for early signs of hypoventilation, airway obstruction, or apnea, i.e., pulse oximetry. Hypoventilation, airway obstruction, and apnea can lead to hypoxia and/or cardiac arrest unless effective countermeasures are taken immediately.
